The fraudulent forms are created by scammers who are attempting to trick their potential victims into submitting their personal, credit card and other sensitive information. Once the scammers receive the information, they will use it fraudulently.

The link to the form above was not created by Nestle, but by scammers. The form has since been taken down by wufoo.com.
