The message claims that you have an opportunity to win Macbook Air, Galaxy S4, or an IPhone 5, if you complete a survey.
If you complete the survey, you will be redirected to another advertisement or taken to an ecommerce website offering you products to buy. You will not receive any of the products the advertisement claims that you have a chance to win.

I took this "Annual Visitor Survey" looking like it was from my bank so I trusted it.I am a little surprised that I DID receive the product of Health Beauty Product for $10.67 post and packaging on 7/10/2014. But I also received Health Beauty Product for $115.62 on 20/10/2014 and then Health Beauty Product for $129.97 on 5/11/2014.I will cancel ALL internet use of my Debit card until I next need it, hoping that these charges will stop before then. Otherwise I need to cancel my Debit card and get a new Debit card.Today I saw Annual Visitor Survey from "bank".co.nz and also "wises.maps.com". I did not select a product. I was looking for the "Health Beauty Product" so I could cancel the "subscription" but this is not possible.
I would recommend contacting your bank and cancelling the card immediately.
Saw this same message popup on computer reboot ( Automatic update I believe). I'm very surprised since I'm very careful about what I download and the sites I visit.
