I just received one two days ago. I almost filled it out until I realized I had to enter my full social security no. no longer allowed by law in NY. Hesitating over this reality, the page timed out and I was sent a second e-mail telling me the page had expired. this email now provided a sign into the document code (which was not needed). Very phishy!! dol phone lines states, NY has allowed them to call you. But they will post any documents or requests on their message board available in your online unemployment account page.
