I don't get it, I've been certifying claims since I apply for UI and PUA since late March. Does it mean I miss a week of certification? I just received one two days ago. I almost filled it out until I realized I had to enter my full social security no. no longer allowed by law in NY. Hesitating over this reality, the page timed out and I was sent a second e-mail telling me the page had expired. this email now provided a sign into the document code (which was not needed). Very phishy!! dol phone lines states, NY has allowed them to call you. But they will post any documents or requests on their message board available in your online unemployment account page. The one time I was contacted by e-mail by NYS UI, it showed in the message box on my sign in page. Also, and this is just my opinion, docusign is used by the kids in addition to google classroom. It is to edit scanned docs, at least that is what we use it for. Be safe and do not fill this form out.
